Workshop registration form template (free)

Running a workshop means planning for the people in the room: how many chairs, how much material, and whether to start with the basics. This workshop registration form asks what you need to know before the day. Page one takes a name, an email, an organization or school, and how people heard about you. Page two asks them to pick one of three sessions from a dropdown, then whether they can stay for the whole thing. Anyone who needs to arrive late or leave early gets a follow-up asking when. It also asks their experience level, whether they will bring their own equipment, what they hope to learn and any accessibility or dietary needs. The thank-you screen tells people you will email the details before their session, so plan to send them. Sign up for free, rename the sessions and share the link.

Free plan, free forever. Sign up and the template opens in your workspace.

How the logic works

  • Shows “When will you arrive or leave?” when “Can you stay for the whole session?” is “I'll need to arrive late or leave early”

What people see after sending

We'll email you the details before your session.

Built in

  • Multiple pages
  • Conditional logic

Who it's for

  • Makers and artists teaching pottery, printmaking or woodwork classes
  • Libraries and community centers hosting free skills sessions
  • Trainers running a seminar or a hands-on course for a company team
  • Schools and universities organizing workshops for students or parents

When to use it

  • When one workshop runs on several dates and people need to choose one
  • When the level of the group changes how you teach
  • When people bring their own tools, laptops or materials
  • When you need numbers before you buy supplies or book a room

Make it yours

  • Rename the three sessions to real days and times, like "Saturday, 10am to 1pm". People skim a dropdown, so a session letter on its own is easy to mix up.
  • Plan around room size. The response limit counts every response to the form, whichever session people pick. If each date has its own room, make a copy of the form for each date, keep one session in each copy and give each copy its own response limit.
  • Charge for a place by adding a Payment block in the editor; the template itself takes no payment. You connect your own Stripe account and set a fixed price or one calculated from the answers, and people pay on Stripe's Checkout page. Each form takes one Payment block. Stripe's processing fees apply, and HornetForms adds no fee of its own.
  • Set a close date and time, for example the evening before, so you can print the list and buy materials. Late visitors then see a closed message you write yourself.
  • Keep the thank-you screen's promise. Turn on the email to respondents, pick the Email question as the address, and write the joining details, parking or what to bring in your own words.
  • Count spare kit before the day. Filter the responses table by "No, I'll need to borrow some" and by experience level, and you know how many extra tools to bring and how to pitch the first hour.

Where the answers go

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

Is there a workshop registration form in PDF or Word?

Yes. This page has a printable PDF in US Letter and A4, plus a Word file you can edit. Keep a few paper copies at the door for walk-ins who would rather not use their phone.

Can I use this as a seminar registration form?

Yes. Change the title and the session names, then drop the equipment question if people only need to sit and listen. The experience level and access needs questions still help you plan a seminar.

What does a workshop registration form sample include?

This sample has 11 questions on two pages: name, email, organization, how they heard about you, preferred session, whether they can stay the whole time, when they will arrive or leave, experience level, equipment, learning goals and accessibility or dietary needs.

Is there a workshop registration Google Form template?

This is a HornetForms template rather than a Google Form, and it can still fill a Google Sheet. The Google Sheets connection works on every plan and adds a row for each person who registers, so a co-host can see the list as it grows. CSV export is free on every plan as well.

Does it work as a training registration form?

It does. For staff training, rename "Organization or school" to team or department, and remove the "How did you hear about the workshop?" question, since everyone heard about it from you.
