Event registration form template (free)

Registration should take a guest one minute, and it should give you everything you need to plan. This event registration form template asks for a full name and an email address, then lets people pick a registration type: General, VIP or Student. It also asks whether they will join the dinner. Only guests who say yes see the dietary requirements question, so everyone else gets a shorter form. Everything fits on one page, which keeps drop-off low on a phone. Every sign-up arrives by email and lands in one list that you can filter and export. Sign up for free, open the template in your workspace and rename the types to match your event.

Free plan, free forever. Sign up and the template opens in your workspace.

How the logic works

  • Shows “Dietary requirements” when “Will you join the dinner?” is “Yes”

What people see after sending

You're registered!

Your place is saved. See you there.

Built in

  • Conditional logic

Who it's for

  • Conference, meetup and workshop organizers
  • Alumni groups and clubs hosting a yearly dinner
  • Schools and universities running open days or student events
  • Community managers who need a head count with food details

When to use it

  • Opening sign-ups for a talk, a workshop or an evening event
  • Splitting attendees into ticket types or groups before the day
  • Planning a meal and needing to know about diets in advance
  • Building a contact list of people who want event updates

Make it yours

  • Rename the registration types. Replace General, VIP and Student with your own options, such as Member, Guest and Speaker, so the answers match how you plan the room.
  • Change the dinner question to any add-on, such as a workshop slot or a shuttle ride. The extra question still appears only for people who say yes.
  • Add a response limit that matches your venue capacity. When the event is full, visitors see a closed message that you write, so no one registers for a seat that does not exist.
  • Use the confirmation email. The form asks for an email address, so you can pick that question in the notification settings and send each guest a note with the date, the address and what to bring.
  • Send registrations to Google Sheets, free on every plan, to get a running list for the door. Each person becomes a row with their type and meal choices.

Where the answers go

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

Who sees the dietary requirements question?

Only guests who answer Yes to the dinner question. People who say No go straight on without seeing it. The choices cover vegetarian, vegan, gluten-free, halal and kosher, and guests can tick more than one.

Can I take payment with this form?

This template is for free registration. If your event has a ticket price, start from the paid event registration template in the gallery. It adds a payment step, and you connect your own Stripe account. Stripe's fees apply.

Do attendees get a confirmation?

They can. The form asks for an email address, and HornetForms can send a confirmation to that address after each sign-up. You choose the email question in the notification settings and write the message yourself. You can also turn on an email to yourself for each new registration.

How do I see who registered?

Every registration lands in the responses table, where you can search and filter by registration type or dinner choice. Export the list as a CSV file, or connect Google Sheets for a live list that updates as people sign up.
