Job application form template with CV upload

Hiring is hard enough without CVs scattered across email threads. This job application form template collects everything in one place over four short pages: contact details, the role, a CV or resume upload and a few questions that tell you more than a CV can. Candidates upload a PDF or Word file up to 10 MB, plus optional work samples, and they never need an account or a sign-in. A hidden source field reads the job board from the link you post, so you can see where your best applicants come from. The form also asks about the right to work and the earliest start date before you book a single call. Sign up for free and the template opens in your workspace, ready for your own roles.

How the logic works

  • Shows “Which role?” when “Which role are you applying for?” is “Other”
  • Makes “Which role?” required when “Which role are you applying for?” is “Other”
  • Shows “Who referred you?” when “How did you hear about this job?” is “Someone who works here”

Built in

  • Multiple pages
  • Conditional logic
  • File upload
  • Hidden fields

Hidden fields it reads from the link: source

Who it's for

  • Small businesses hiring without an applicant tracking system
  • Founders filling their first few roles
  • Office and HR managers who post on several job boards at once
  • Shops, cafés and service companies that hire all year round

When to use it

  • Posting a role on your careers page or LinkedIn
  • Sharing one form across Indeed and other job boards
  • Taking applications on a tablet at your front desk
  • Building a pool of candidates for roles you fill often

Make it yours

  • Replace the sample roles in the dropdown (software engineer, product designer, customer success manager) with your open positions. Keep Other: it shows a short text box when someone picks it.
  • Post a different link on each job board, like ?source=indeed and ?source=linkedin. The hidden source field stores it with each application, so your spreadsheet shows which board works.
  • Leave the cover letter optional. The question "What's something your CV doesn't show?" often tells you more, and it costs the candidate two minutes, not an evening.
  • If a role can't offer visa sponsorship, say so in the job post too, so the right to work answer never comes as a surprise.
  • Connect Slack so the hiring team sees each new application as it arrives, free on every plan.

  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

Can candidates upload a CV without a Google account?

Yes. Candidates upload their CV straight into the form, with no Google account and no sign-in. The CV question takes a PDF or Word file up to 10 MB, and the cover letter question takes up to three files. The files sit with the rest of the answers in each response.

How do I know which job board an applicant came from?

The template has a hidden field called source. Add it to the link you post, like ?source=indeed, and every application from that link carries the value. For people who arrive another way, the form also asks how they heard about the job, and who referred them if they pick someone who works here.

Can I use one form for several roles?

Yes. The role page starts with a dropdown of your open roles, and picking Other asks which role in a short text box. You can also choose the role from the link with a prefill parameter, so each job post opens the form with its role already picked.

Can people apply from their phone?

Yes. The form is split into four short pages, so it fits a phone screen without endless scrolling. Candidates can answer on the bus and attach a CV saved on their phone.

What do candidates see after they apply?

A short thank-you that says you'll reply by email once you've reviewed the application, whether or not they move forward. Change it to match your process, and turn on a confirmation email so they keep a copy of what they sent.
