Time off request form template (free)

People ask for time off in a dozen ways: a hallway chat, a text, a sticky note on the fridge. Then someone forgets. This time off request form template gives every request the same shape. Employees pick the type of leave, the first and last day, and whether they need full days, a half day or a few hours. They also say who covers for them and whether they can be reached. You get each request by email the moment it's sent, and every request stays in one list instead of five inboxes. It works for PTO, vacation, annual leave, sick days and holiday requests, so you need one link, not five forms. Sign up for free, open the template in your workspace and rename anything to match your policy.

How the logic works

  • Shows “What kind of time off?” when “Type of time off” is “Other”
  • Makes “What kind of time off?” required when “Type of time off” is “Other”
  • Shows “From” when “Full days or part of a day?” is “A few hours”
  • Makes “From” required when “Full days or part of a day?” is “A few hours”
  • Shows “Until” when “Full days or part of a day?” is “A few hours”
  • Makes “Until” required when “Full days or part of a day?” is “A few hours”
  • Shows the note “Your last day off is before your first day off. Please...” when “Last day off” is less than “First day off”
  • Hides the submit button when “Last day off” is less than “First day off”

What people see after sending

Request sent

Your manager will check it and reply by email. Until then, these dates are pending.

Built in

  • Multiple pages
  • Conditional logic
  • Hidden fields

Hidden fields it reads from the link: employee_id

Who it's for

  • Small business owners who approve leave themselves
  • HR and office managers handling requests for a whole team
  • Team leads who need to plan cover before saying yes
  • Shift teams in shops, salons and cafés, where a half day matters

When to use it

  • Booking vacation or annual leave weeks ahead
  • Asking for a half day or a few hours for an appointment
  • Logging a sick day, bereavement or jury duty
  • Planning parental or unpaid leave with a manager

Make it yours

  • Rename the leave types to match your policy. If your team says PTO, change the first choice to PTO so nobody wonders which option to pick.
  • Swap the team or department question for a dropdown of your real teams, so the answers sort cleanly in a spreadsheet.
  • Add an employee ID to the link you share. The template already reads an employee_id hidden field, so every request carries the ID without anyone typing it.
  • Connect Google Sheets to keep a running time off log, free on every plan. Each request becomes a new row with its dates and leave type.
  • Keep the date check. The form hides the send button when the last day comes before the first day, which saves a round of emails.

Where the answers go

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

Is this the same as a PTO request form?

Yes. PTO, vacation, annual leave and holiday requests all ask the same things: what kind of leave, which days and who covers. The template lists vacation, sick leave, personal days and more in one dropdown, so one form handles them all. If your company says PTO, rename the first choice and you're done.

Can employees ask for a half day or just a few hours?

Yes. The form asks whether they need full days, a morning, an afternoon or a few hours. Pick a few hours and two time questions appear, From and Until, and both become required. For full days, those questions stay hidden.

Who gets the request when someone sends it?

You do, once you turn on an email to yourself for each new response. It works on every plan. You can also send the employee a confirmation email, because the form asks for their work email. To keep a log, connect Google Sheets and each request becomes a new row.

Do employees need an account to fill it in?

No. Anyone with the link can fill it in on a phone or laptop without signing in. Share it in your team chat, add it to your handbook or print its QR code for the staff room.

How should employees count the days they need?

The form asks for working days only, with a half day as 0.5. Asking people to count keeps weekends and public holidays out of the total, and you can check the number against the dates before you approve.
