Cleaning client intake form template

Every cleaning quote starts with the same questions, and most of them get asked twice. This cleaning client intake form template asks once, in the order that changes your price: the type of place, bedrooms and bathrooms, square footage and how it looks right now. Then it asks which clean they need, any extras like inside the oven or fridge, and how often you'll visit. The last page covers the day itself: pets, who will be home and how your team gets in. Clients can add photos of the rooms, so you can often quote without a walkthrough. The thank-you screen shows a rough price range worked out from their answers, so nobody waits a day for a ballpark. How the logic works

  • Shows “Business name” when “What type of place is it?” is “Office or commercial space”
  • Shows “Move date” when “Which clean do you need?” is “Move-in or move-out clean”
  • Shows “Tell us about them” when “Any pets at home?” is “Yes”
  • Makes “Tell us about them” required when “Any pets at home?” is “Yes”
  • Shows “How will we get in?” when “Will someone be home during the clean?” is “No” or “Will someone be home during the clean?” is “Sometimes”
  • Makes “How will we get in?” required when “Will someone be home during the clean?” is “No” or “Will someone be home during the clean?” is “Sometimes”
Show the calculation rules (11)
  • Adds 25 to the calculated field “estimate_low” when “Bedrooms” is “2”
  • Adds 50 to the calculated field “estimate_low” when “Bedrooms” is “3”
  • Adds 75 to the calculated field “estimate_low” when “Bedrooms” is “4”
  • Adds 100 to the calculated field “estimate_low” when “Bedrooms” is “5 or more”
  • Adds 15 to the calculated field “estimate_low” when “Bathrooms” is “1.5”
  • Adds 30 to the calculated field “estimate_low” when “Bathrooms” is “2”
  • Adds 45 to the calculated field “estimate_low” when “Bathrooms” is “2.5”
  • Adds 60 to the calculated field “estimate_low” when “Bathrooms” is “3”
  • Adds 90 to the calculated field “estimate_low” when “Bathrooms” is “4 or more”
  • Multiplies the calculated field “estimate_low” by 1.25 when “How would you describe it right now?” is “Needs extra attention”
  • Multiplies the calculated field “estimate_low” by 1.5 when “How would you describe it right now?” is “Hasn't been cleaned in a while”

Built in

  • Multiple pages
  • Conditional logic
  • File upload
  • Calculated field
  • Hidden fields

Hidden fields it reads from the link: source

Who it's for

  • House cleaners and maid services quoting new clients
  • Small cleaning companies running a few crews
  • Office cleaners, since the form asks for a business name when it's an office
  • Move-out and after-builders cleaning specialists

When to use it

  • A new lead asks how much a clean costs
  • Booking a move-in or move-out clean around a fixed move date
  • Setting up a weekly or every-two-weeks schedule
  • Pricing a deep clean for a place that hasn't been cleaned in a while

Make it yours

  • Set the estimate to your prices. The template adds to the low estimate for each extra bedroom and bathroom, and multiplies it when a place needs extra attention. Change those amounts in the logic to match what you charge.
  • Never collect alarm or lockbox codes in a form. The access question already tells clients you'll arrange codes by phone, and it only appears when nobody will be home the whole time.
  • Add a dropdown of the towns you cover, so you can tell at a glance whether a lead is in your area.
  • Share the form from your Google Business Profile with ?source=google on the end of the link. The hidden source field records where each lead came from.
  • Send each new lead to Slack or Discord, or keep a quotes sheet in Google Sheets, free on every plan.

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

What should a cleaning client intake form ask?

The answers that change your price and your day: the type and size of the place, bedrooms and bathrooms, its current condition, the clean they want, extras, how often, pets and access. This template asks all of that in 25 questions over four short pages, and most of them are quick taps, not typing.

Can the form give clients a price?

It gives a rough range. A calculated field adds to the estimate for each extra bedroom and bathroom and raises it for places that need extra attention. The thank-you screen shows the range, and you still send the real quote after you read the answers. Set the amounts to your own rates in the editor.

How do I quote a cleaning job without a walkthrough?

Ask for photos. The last page has an optional photo upload with a note that a few photos help you quote without a visit. Together with the size and condition answers, that's often enough for a standard clean. For a deep clean or a clean after building work, you may still want to see the place first.

Is it safe to ask clients how we get in?

Ask how, never for the codes. The access question only shows when nobody will be home for the whole clean, and its help text asks clients not to type alarm or lockbox codes. Arrange those by phone, so they never sit in a form response or an email.

Does it work for office cleaning too?

Yes. Pick "Office or commercial space" and a business name question appears. For regular office contracts, add questions about opening hours and floor area, and use logic to hide the bedroom question for those clients.
