Client intake form template (free)

The first call goes better when you already know what the client wants. This client intake form template sorts new enquiries before you reply. It asks for a name, an email, a business name and a website, then which services the person needs: design, a website, copywriting, ads, development or coaching. It asks what they hope to achieve, what budget they have in mind, when they want to start and how they found you. If they have worked with a freelancer or agency before, an extra question asks what worked and what didn't. You read one tidy answer set instead of a long email thread. Sign up for free and rewrite the options to match your services.

Free plan, free forever. Sign up and the template opens in your workspace.

How the logic works

  • Shows “What worked well, and what didn't?” when “Have you worked with a freelancer or agency before?” is “Yes”

What people see after sending

Thanks, got it!

I'll read this properly and come back to you within two business days.

Built in

  • Conditional logic

Who it's for

  • Freelance designers, developers and writers who get enquiries by email
  • Small agencies that screen leads before booking a discovery call
  • Consultants and coaches who want to know the goal before the first session
  • Studios that need budget and timing in hand before they quote

When to use it

  • Replacing a contact page that only collects a short message
  • Qualifying leads from a website, a portfolio or a social profile
  • Gathering scope and budget before you write a proposal
  • Learning which channels bring in the best clients

Make it yours

  • Replace the service checkboxes with your real services. Clients tick what they need, and you see at a glance whether the work fits you.
  • Adjust the budget ranges to your prices. Keep a Not sure yet option, so people who have no number in mind still finish the form.
  • Edit the thank-you message to promise a reply time you can keep. The template ships with two business days, so change it if you answer faster or slower.
  • Send new enquiries to Slack, free on every plan, so the whole team sees a new lead the moment it arrives. You can also turn on an email to yourself for each new enquiry.
  • Use the how did you hear about me answers. After a few months, filter the responses to see which source sends the clients you most want.

Where the answers go

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

What should a client intake form ask?

Ask for what changes your answer: the services needed, the goal, a budget range, the start date and past experience with similar work. Contact details come first. This template covers all of that in 11 questions on one page, and only the essentials are required.

Is this the same as a client onboarding questionnaire?

Not quite. An intake form screens a new enquiry before you agree to work together. An onboarding questionnaire comes after the client says yes and collects the details you need to start. Many teams use both, one after the other.

Why ask about budget in the first form?

A range saves both sides time. If the budget is far below your minimum, you know before you spend an hour on a call. The question is optional and has a Not sure yet choice, so it never blocks someone who is still planning.

Which question appears only for some clients?

The question about what worked well and what didn't. It shows only when a client says they have worked with a freelancer or agency before. First time clients skip it, so their form stays short.
