New employee information form (free template)

The week before someone starts, you need the same handful of facts every time. Their legal name for payroll. A personal email that works before the work account exists. A home address for the laptop, and someone to call in an emergency. This new employee information form collects all of it in two short pages. Page one covers the person and their emergency contact. Page two covers kit and practical things: laptop preference, desk equipment, T-shirt size, dietary needs for the first-week lunch, and any workplace adjustments. Send the link once they accept the offer and the answers arrive before day one, so the laptop is ready and nobody spends the first morning filling in paper. Sign up for free and the template opens in your workspace, ready to edit.

  • Shows “Which distribution, and do you need admin rights?” when “Laptop preference” is “Linux”
  • Shows “What adjustments would help?” when “Do you need any accessibility or workplace adjustments?” is “Yes”

What people see after sending

All set. See you on day one!

Your manager will send an agenda for your first week, and your kit will be waiting.

Built in

  • Multiple pages
  • Conditional logic

Who it's for

  • Small business owners who handle hiring and setup themselves
  • Office and HR managers getting several new starters ready each month
  • IT or operations people who order laptops and desk kit before day one
  • Founders hiring their first few people with no HR system yet

When to use it

  • Right after a candidate accepts the offer, with the welcome email
  • Before a remote hire's start date, so the laptop ships to the right address
  • When a contractor moves to a staff role and you need fresh details
  • When you rehire someone and want to check their details are current

Make it yours

  • Keep tax, bank and ID numbers out of this form. Collect them through your payroll provider or HR system. They handle that paperwork. This form is for the practical details that get someone ready for day one.
  • Swap the laptop choices for the machines you actually buy. If everyone gets the same laptop, delete the question and the Linux follow-up with it.
  • Trim the desk kit list to what you hand out. A remote team might add a chair budget question; an office team might drop the laptop stand.
  • Connect Slack, or turn on email notifications, so IT sees each new starter's kit request the moment it arrives instead of on day one.
  • Talk to anyone who picks "I'd rather discuss it privately" for adjustments. It means they'd like to talk it through one to one, so book a short call before the start date.

Where the answers go

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

What should a new employee information form include?

Most include the legal and preferred name, personal email and phone, home address, start date and an emergency contact. This template adds the practical side: laptop preference, desk kit, dietary needs for team events and a question about workplace adjustments, so setup is done before the first morning.

Should I ask for bank details or tax numbers on this form?

Keep them out. Payroll, tax and work-eligibility paperwork has its own official forms and rules, so handle it through your payroll provider or HR system. This form leaves those fields out on purpose and sticks to the details that help you get ready for day one.

Is there a printable PDF version of the form?

Yes. Download the new employee information form as a PDF for US Letter or A4, or as a Word file you can edit and add your logo to. The paper version has the same questions in the same order as the online form.

Is this new employee information form template free?

Yes. The template works on the Free plan, which is free forever. Sign up, open the template, edit the questions and send the link. New hires fill it in on any phone or laptop without signing up or signing in, which matters because their work account usually does not exist yet.

Is a new employee details form the same thing?

Yes. Some teams call it a new employee details form, a new hire information form or a starter form. They all collect the same core facts before day one: name, contact details, start date and an emergency contact.

  • Reference check form template (free)

A free reference check form referees fill in on their phone: how they worked with the candidate, four ratings, strengths and a hire-again question.

    14 questions on one page

  • Time off request form template (free)

A free time off request form for PTO, vacation, sick days and half days. Staff send it from any phone, and you get each request by email.
