Partner program application form (free template)

A partner program application form lets you say yes to the right affiliates and agencies without a call with every applicant. This template starts with the basics: full name, work email, company or brand, a website or profile link and the kind of partner they want to be. Affiliates then see questions about audience size and where they would promote you. Resellers say which regions they sell in and whether they need local invoicing, and technology partners say what they want to integrate with. Everyone describes their audience, how they would promote or use the product, their experience, expected referrals per month and other programs they belong to. The form has 14 questions on two pages, and each applicant answers only the ones that fit. Sign up for free, open the template and put the link on your partners page.

How the logic works

  • Shows “How big is your audience?” when “What kind of partner are you?” is “Affiliate”
  • Shows “Where would you promote us?” when “What kind of partner are you?” is “Affiliate”
  • Shows “Which regions do you sell in, and do you need local invoicing?” when “What kind of partner are you?” is “Reseller”
  • Shows “What would you want to integrate with?” when “What kind of partner are you?” is “Technology integration”

What people see after sending

Application received!

Our partnerships team reviews applications weekly and will email you either way.

Built in

  • Multiple pages
  • Conditional logic

Who it's for

  • SaaS founders opening an affiliate program for the first time
  • Partnership managers who review agency and reseller applications
  • Course creators and newsletter writers recruiting affiliates of their own
  • Product teams that want integration partners to apply in one place

When to use it

  • As the "Apply" button on your partners or affiliates page
  • When partner requests arrive by email and get lost in the inbox
  • Before you pay for affiliate software, to see how many people apply
  • When you reopen a paused program and want a fresh list of applicants

Make it yours

  • Ask how they will disclose the link. In the US, the FTC's Endorsement Guides say a connection between an endorser and the seller that might change how people weigh the endorsement, and that the audience would not expect, "must be disclosed clearly and conspicuously", and that material connections "can include monetary payment" (16 CFR 255.5, eCFR, checked 2026-10-28). Add one question asking how they will tell their audience they are paid for referrals, and check the rules where you sell.
  • Keep links and payments in your affiliate tool. This form handles the application. Once you approve someone, set up their referral link and payment terms in the affiliate or billing software you already use.
  • Review applications in a sheet. Connect Google Sheets on any plan and each application becomes a row. Add your own columns for the decision and the date you replied, then sort by partner type.
  • Check the link before the numbers. Audience size is self-reported. Open the website or profile the applicant gave and read a few recent posts before you trust the dropdown answer.
  • Pause the program without deleting the form. Set a close date or a response limit, then write your own closed message, such as "Applications reopen in March". Visitors see that message instead of the form.
  • Reply to everyone. The thank-you screen says the team reviews applications weekly and will email either way. Keep that promise, or rewrite the line to match how often you really check, and turn on an email to yourself for each new application so none sits unread.

Where the answers go

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

What should an affiliate application form ask?

Contact details, a link to their site or profile, who their audience is, roughly how big it is and where they would promote you. Add how they would describe the product and which other programs they belong to, so you can spot a good fit quickly.

What is the difference between an affiliate and a partner?

An affiliate usually shares a link and earns money for the sales or sign-ups it brings. Partner is the wider word: agencies that set up your product for clients, resellers that sell it in their region and technology partners that build an integration. This form asks each type different questions.

What is a partner program?

It is a set of terms for working with businesses that bring you customers, from affiliates and resellers to agencies and integration partners. Most programs start with an application, so you can check each partner suits your product before you share links or terms.

How do you pause partner program applications?

Set a close date or a response limit on the form, on any plan, and write a short closed message. Anyone who opens the link sees your message, such as when applications reopen, instead of the form. Clear the date or raise the limit when you are ready to accept applicants again.

How do I start an affiliate program?

Decide what you reward and how much, pick software to track referral links, write simple terms, then open applications with a form like this one. Start small: approve a handful of partners whose audience matches your customers, and learn from them before you invite more.
