IT support request form template (free)

When the printer dies or a login stops working, people message whoever looks techy. The details get lost, and the same problem gets reported three times. This IT support request form template gives everyone one place to ask for help. It asks for a name and email, a category and a priority. Pick Urgent, and the form asks for a phone number for a callback and makes it required. Then people describe the issue and can attach up to three screenshots. Seven questions keep it short enough to fill in on a phone while the laptop is frozen. Sign up for free, open the template and rename the categories to the problems your team really has.

How the logic works

  • Shows “Phone number for urgent callbacks” when “Priority” is “Urgent”
  • Makes “Phone number for urgent callbacks” required when “Priority” is “Urgent”

What people see after sending

Request received

Our support team will get back to you as soon as possible.

Built in

  • Conditional logic
  • File upload

Who it's for

  • The office manager who also looks after the laptops, Wi-Fi and printers
  • Small IT teams and outside IT contractors who support several offices
  • Agencies and freelancers who handle tech requests for their clients
  • Schools and nonprofits where one person handles every tech problem

When to use it

  • As the one link staff use for any tech problem, from email to printers
  • For new account and access requests when someone joins the team
  • When an issue can't wait and you need a number to call back
  • For hardware requests, such as a new keyboard or a second monitor

Make it yours

  • Rename the categories. The template starts with Account, Billing, Technical issue and Other. For an office, try Email, Wi-Fi and network, Printer, Laptop or phone, Software and New starter setup. Short, plain names help people pick the right one.
  • Keep the urgent rule, and say what urgent means. Pick Urgent and the callback number appears and becomes required, so you can phone the person right away. Add help text to Priority, such as "Urgent means I can't work at all", so not every request lands there.
  • Tell people never to type a password into the form. Add a short note at the top that says so. If someone needs a reset, call them back or sort it out in person. Then no password ends up in a spreadsheet or a chat message.
  • Ask for a screenshot of the error. The upload question takes up to three images, 10 MB each, which covers most error messages. A picture of the exact wording saves a round of "what does it say?" messages.
  • Add a hidden field for the office or site, and fill it in the link you share in each location. Every request then shows where it came from, and nobody has to remember which building they're in.
  • Send new requests where you'll see them. Connect Slack or Discord, free on every plan, and each one posts to your IT channel with its category and priority. Turn on a confirmation email to the requester as well, written in your words, and pick the Email question as the address, so they know you have it.
  • Connect Google Sheets to keep a running log. Each request becomes a row, so after a month you can see which category comes up most, and maybe replace that old printer for good.

Where the answers go

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

What is an IT service request?

It's a request for help or for something new from whoever runs IT: a fix for a problem, a new account, access to a shared folder or new hardware. A form gives every request the same details, so you can filter them by category and priority.

Can I use this as an IT help request form?

Yes, that's its first job. Each request arrives as a response with its category, priority, description and screenshots. You can filter and export responses, send them to Slack, Discord or Google Sheets, and reply to the person from your own email or phone.

Is there an IT service request form template in Word or PDF?

Yes. Download a printable PDF in US Letter or A4, or a Word file you can edit. Paper helps at a front desk, or for staff who can't get online, which is often exactly when they need IT.

How do I submit an IT request with this form?

Open the link your team shared, add your email, pick a category and a priority, then describe what happened and what you were trying to do. Attach a screenshot of any error. If you pick Urgent, leave a phone number so someone can call you back.

Can I use it for IT maintenance requests?

Yes. Add a Maintenance choice to the category list, for planned jobs such as a laptop swap or a software update, and a date question for when it suits the person. A show rule can reveal that date question only for maintenance.
