Creative request form template (free)

Requests for marketing work tend to arrive as "can you make something for the launch?" with no deadline and no files. This creative request form template asks for the details up front. The requester picks what they need, from a blog post or landing page to a video script or ad creative, and gives it a working title. Then they say what it should achieve, who it's for, where it will run and when they need it. A priority question separates nice-to-have work from a campaign that depends on it. Ad requests get one extra question about platform and sizes. Brand assets, screenshots and links arrive in the same response. Sign up for free, open the template and rename the choices to match what your team really makes.

How the logic works

  • Shows “Ad platform and sizes” when “What do you need?” is “Ad creative”

What people see after sending

Request logged!

The marketing team will confirm a delivery date shortly.

Built in

  • Conditional logic
  • File upload

Who it's for

  • In-house marketing and design teams that take requests from sales, product and leadership
  • Solo designers and content leads tired of requests that live in DMs
  • Agencies that want clients to send new work through one link
  • Nonprofit comms teams fielding asks from every program at once

When to use it

  • As the front door for every design, copy or video request
  • At quarterly planning, to collect what each team wants made
  • Before a launch or event, when several asks land in the same week
  • When a request comes with files, such as logos, screenshots or data

Make it yours

  • Read the goal before you talk about format. The form asks "What should this achieve?" straight after the working title (and the ad question, for ad requests) for a reason. A request for three Instagram posts sometimes turns out to need one good email. Agree on the format once you both know what success looks like.
  • Edit "What do you need?" so it lists the work your team takes on. Drop video scripts if you never write them, and add pitch decks or event banners if you do. Then copy the idea behind the ad question: a show rule can ask for a slide count only when someone picks a deck.
  • Put your lead times in the help text of "Needed by", for example two weeks for a landing page and three days for a social post. People pick a more realistic date when they can see the rule right under the question.
  • Big video files can outgrow an upload question. On the Free plan each file can be up to 10 MB, and one upload question takes up to 10 files. For raw footage, ask for a shared folder link in "Brief, docs or references" instead.
  • Send new requests where your team already plans. Connect Slack or Discord so each one posts to your channel, or Notion or Google Sheets so each one becomes a page or row with its type, deadline and priority. All four are free on every plan.
  • The thank-you screen says the team will confirm a delivery date shortly. Keep that promise, or reword it to what you'll really do, such as a reply within two working days. Turn on an email to yourself for each new response, so nothing sits unread.

Where the answers go

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

What is the difference between a creative request form and a creative brief?

A request form is what someone sends to ask for work: what they need, why and by when. A creative brief is what the team writes once it takes the job on, with the message, tone and must-haves. Many teams start the brief from the request, so a clear form saves time twice.

Can I use this as a graphic design request form?

Yes. Rename the choices in "What do you need?" to design work, such as social graphics, print, slides or banners, and keep the upload for logos and examples. The ad question already asks for platform and sizes, which designers need before they open a file.

Is there a marketing request form template in Word or PDF?

Yes. There is a printable PDF for US Letter and A4 and a Word file you can edit. Most teams use the online form day to day and keep the paper version for planning meetings or for people who like to sketch a request first.

What should a creative request include?

At least the type of work, a working title, the goal, the audience, where it will run, the deadline and any files or links. This template asks for all of them. Add a budget or cost center question if your team charges work back to other departments.

Where can I see creative request form examples?

Open the live preview to see this form the way a requester does. Pick Ad creative in the first dropdown and watch the platform and sizes question appear. The question list on this page shows every field, choice and rule in the template.
