Pet sitting intake form template (free)

Taking care of someone's pet means knowing a lot before the first visit. This pet sitting intake form template gathers it in six short pages. Owners start with their own details, then the booking: the kind of care, the first and last day and the visits each day. Next comes the pet, with the feeding routine, medicines and quirks. A second pet gets its own page, and the page disappears when there is no second pet. Then the vet, an emergency contact, and how you will get into the house. How the logic works

  • Shows the note “The last day of care is before the first day. Please check...” when “Last day of care” is less than “First day of care”
  • Hides the submit button when “Last day of care” is less than “First day of care”
  • Goes to the page “Vet and emergencies” when “Do you have another pet for us to look after?” is “No”
  • Shows “Neighbour's name and address” when “How will we get in?” is “Spare key with a neighbour”
  • Makes “Neighbour's name and address” required when “How will we get in?” is “Spare key with a neighbour”

What people see after sending

Thanks, we've got everything

We'll read through your answers and call you to arrange keys and a meet-and-greet before the first visit.

Built in

  • Multiple pages
  • Conditional logic
  • File upload
  • Hidden fields

Hidden fields it reads from the link: source

Who it's for

  • Independent pet sitters and dog walkers who take new clients each week
  • Small pet care businesses that book drop-in visits and overnight stays
  • House sitters who look after animals as part of the job
  • Friends and family who pet sit often and want it written down

When to use it

  • Sending the form after a new client says yes and before the meet and greet
  • Collecting feeding and medicine details in writing instead of by text
  • Booking drop-in visits, dog walks, overnight stays or house sitting
  • Learning where leads, litter and spare supplies are kept

Make it yours

  • Keep the warning on the booking page. If the last day falls before the first day, a note asks the owner to check the dates and the submit button stays hidden, so you never get a booking that runs backward.
  • Leave the door code request out of the form. The key question already asks owners not to type alarm or lockbox codes, and you can arrange those by phone at the meet and greet.
  • Edit the care options to match what you sell. If you only offer walks and drop-ins, delete the other two choices and the form stays tidy.
  • Ask for a photo of the pet. The upload is optional, and a clear picture helps you recognise a shy cat or a dog that slips out of the door.
  • Turn on email notifications and read each answer set before you call. The thank-you message tells the owner you will phone to arrange keys.

Where the answers go

Every response lands in your HornetForms inbox. From there, send it wherever your team already works.

  • Turn on an email to yourself for each new response, on every plan
  • A confirmation email to the person who filled it in, when the form asks for their email
  • A new row in Google Sheets, a message in Slack or Discord, or a page in Notion, free on every plan
  • Connect with Zapier, or send signed webhooks to n8n, Make or your own app
  • CSV export on every plan, and Excel export on Pro and Business

Questions people ask

What happens if the owner has only one pet?

They answer no to the question about another pet and the form skips the second pet page. They land on the vet and emergency page right away, so the form stays short for most clients.

Why does the form not ask for door codes?

Codes should not sit in a form response. The key question asks owners to leave alarm and lockbox codes out and promises to arrange them by phone. You then share them in a way you and the owner agree on.

What if a client picks the wrong dates?

When the last day is earlier than the first, a note appears asking the owner to check, and the submit button hides. As soon as they fix the date, the button returns and they can send the form.

Can owners add more than two pets?

Yes. The second pet page ends with a long answer for the rest of the pets, where owners write each name, feeding routine and medicine. A client with several animals can also send one form per pet.
