HTML form snippet pack
From https://hornetforms.com/blog/html-form-generator

What is in here
- html-form-contact.html: a contact form (name, email, topic, message, how to reply, optional phone).
- html-form-signup.html: a sign-up form for launch news (optional first name, email, interests,
how they heard about you, a consent tick box).
- html-form-feedback.html: a feedback form (a 1 to 5 rating, what worked, what to change,
  follow-up permission, optional email).

Each file is a full web page. Open it in any browser to see the form. Each one passed the
W3C Nu HTML Checker with no errors, uses a label for every field and needs no JavaScript.

Before you put one on your site
1. Change the action URL (https://example.com/your-form-handler) to the address of your own
form handler. The HTML only draws the form. A program on a server has to receive the
answers, check them again and send or store them. Without that, the answers go nowhere.
2. Edit the labels, options and button text. Keep each label's "for" equal to its field's "id".
3. Copy the <style> block into your stylesheet, or replace it with your own styles.
4. Spam: your server should drop any response that fills in the hidden "website" field
(the honeypot) and any response that comes back in under about three seconds
(the time check: your server writes the time it sent the page into "started_at").

Prefer not to run a server? Build the form in a form builder and paste its embed code
instead. The blog post above shows both ways.
